Chronic Disease Management

We participate in the Chronic Disease Management programme in conjunction with the HSE. 

If you have a Chronic Disease, you may be asked to attend twice yearly for a check-up with our nurse and GP.

Conditions that are included in the programme are:

• Asthma
• Diabetes Type 2
• Chronic Obstructive Pulmonary Disease (COPD)
• Cardiovascular Disease including:
• Heart Failure
• Ischaemic Heart Disease
• Cerebrovascular Disease (Stroke/ Transient Ischemic Attack (TIA))
• Atrial Fibrillation
